COVID 19 Information
In conforming to the governor’s Executive Order 149 specifies that high school sports activities under the jurisdiction of the New Jersey State Interscholastic Athletic Association (“NJSIAA”) may resume in accordance with reopening protocols issued by NJSIAA, which shall consider NJ Department of Health (DOH) guidance in issuing these protocols. The following guidelines are for schools wishing to participate in athletic events The accuracy of completing these forms is imperative to the success of the athletic programs at Monmouth Regional High School.
Please choose the daily form for the practice you will be participating in and complete on line on the day of, prior to arriving to school.

Student athletes are not permitted to participate in workouts if there is a "YES" answer on the questionnaires. In this case, the student athlete must be cleared by a doctor before they can participate ( a doctors' note is required). Additional guidelines and specific timing for subsequent phases are pending, and details will be shared no less than two weeks before the next phase begins. NJSIAA continues to emphasize that all dates are subject to revision.

Prior to scheduling an appointment all student athletes parents are required to complete the revised Health History Update (Click here for the link): Health History Update
The Health History Update Questionnaire has been updated to include 2019 Novel Coronavirus (COVID-19) related-questions. Pursuant to N.J.S.A. 18A:40-41.7(b), the Health History Update Questionnaire is to be completed and signed by a student-athlete’s parent or guardian when the student-athlete’s last physical examination was completed more than ninety (90) days prior to the first day of official practice in an athletic season. Once completed, the Health History Update Questionnaire shall be reviewed by the school nurse and, if applicable, the school athletic trainer. Pursuant to N.J.S.A. 18A:40-4, if an answer to a question on the Health History Update Questionnaire is yes, the certified school nurse and school physician (or other designated medical professional) shall determine whether additional medical attention and/or further evaluation is necessary. All student athletes participating in a sport at MRHS are required to complete the Updated Health History Form. All HEALTH HISTORY FORMS and or PRIVATE PHYSICALS MUST BE SUBMITTED AND RECEIVED TO THE HEALTH OFFICE BY HOMEROOM THE DAY BEFORE THE CORRESPONDING PHYSICAL EXAM APPOINTMENT. NO EXCEPTIONS.

Welcome to Monmouth Regional. I have been with the district since January 2000. I have 17 years of Health and Physical Educaiton teaching experience. , I was Assistant Principal for 2 years and currently serving as the Athletic Director and Health & Physical Education supervisor for the past 13 years. I was a former boys and girls soccer coach for 15 years as well as an Alumni from Monmouth Regional.
