Prior to scheduling an appointment, all student athletes' parents are required to complete the revised Health History Update (Click here for the link): Health History Update : Preparticipation Physical Evaluation Form: Physical Exam Form
All students planning to participate in sports must have one comprehensive sports physical per year. According to the N.J.A.C 6A:16-2.2 et.seq. each candidate for a school athletic team must have a medical examination within 365 days prior to the first practice session and a health history update within 90 days of the first practice session. Once completed and signed appropriately, this entire paper portion must be submitted to the Health Office mailbox in the main office to be considered for sports participation. All private physical paperwork must be reviewed by our school physician ONLY at the dates shown above. The doctor’s office does not accept paperwork from parents. The school nurse and school physician will then evaluate the examination. Any omissions may delay the pre-participation process.
The Health History Update Questionnaire has been updated to include 2019 Novel Coronavirus (COVID-19) related-questions. Pursuant to N.J.S.A. 18A:40-41.7(b), the Health History Update Questionnaire is to be completed and signed by a student-athlete’s parent or guardian when the student-athlete’s last physical examination was completed more than ninety (90) days prior to the first day of official practice in an athletic season. Once completed, the Health History Update Questionnaire shall be reviewed by the school nurse and, if applicable, the school athletic trainer. Pursuant to N.J.S.A. 18A:40-4, if an answer to a question on the Health History Update Questionnaire is yes, the certified school nurse and school physician (or other designated medical professional) shall determine whether additional medical attention and/or further evaluation is necessary. All student athletes participating in a sport at MRHS are required to complete the Updated Health History Form. All HEALTH HISTORY FORMS and or PRIVATE PHYSICALS MUST BE SUBMITTED AND RECEIVED TO THE HEALTH OFFICE BY HOMEROOM THE DAY BEFORE THE CORRESPONDING PHYSICAL EXAM APPOINTMENT. NO EXCEPTIONS.
