Calling out

  • Below is a friendly reminder of the call out procedures. 

    Call extension 1153 and leave a message for Tamera Hayes even if you do not need coverage.


    Please note that all calls MUST be BEFORE 6am.

    Contact your supervisor.

    Please submit your leave request form through your employee portal. 

    Calling out the day before and/or after a holiday:

    Leave of absence for 3 or more days: 

    • Follow the calling out procedures.

    • Please contact the nurse's office via phone or email, they will advise you of what is needed. The nurses will contact Attendance, you do not have to contact the sub coordinator or call out every day after speaking with the nurse.

  • Please fill out the leave request form in your staff portal

  • Notify your supervisor as well

  • To schedule a day in advance please use the staff portal to fill out a Leave Request Form

  • All employees must fill out a Leave of Absence form on the staff portal

  • If you will be out over 3 days, you are required to produce a doctor’s note (Turn in to nurses office only)

  • If you will be out over 2 weeks, you are required to have prior board education approval

 Putting in for leave request:

  • You must contact sub coordinator x1153

  • Employee tells supervisor

  • Employee fills out Leave Request Form in Genesis Employee Portal ( See Page 29 )

    • Upon board approval, a letter is sent to the employee. If more time is needed, need to let supervisor, administration and nurse know as soon as you are given this information. Notice, up to 30 days in advance, may be required for any long term leaves of absence.

    • Upon return, the employee needs a note that you are fit to return to work and if you need modifications upon your return (1/2 days, special chair etc.)
